so ungrateful

i was called by my boss last Friday to his office, and he handed me a letter. i wasn't really sure to whom it was addressed to but definitely not directly to me. but anyway, i was directly involved in that matter. oh God, it's from a lawyer.

Boss told me, it's regarding a dengue shock syndrome patient that i referred to HTAR, Klang recently. Initially, i thought that something bad had happened to her, alas it was worse than that. if it is difficult to accept a dengue mortality, this is more difficult to be dealt with. an ungrateful, snobbish, arrogant human being!!no, i don't blame the patient (i can say she's a nice girl), i am blaming the parents/relative/asshole who did this!

this patient was admitted for dengue fever, Day 5 of illness. when i reviewed the next day, she was still in febrile phase, but tachycardic and bit hypotensive. but managed to give her fluid resus. Hematocrit were less than 35%, platelet in decreasing trend, but still more than 50, white cell in increasing trend. there was no warning sign.

however the next day, she developed symptoms. started to have epigastric pain and intermittent SOB. temperature just touched the baseline, tachycardic still and hypotensive. when i traced the FBC, hematocrit was 37.7%, but platelet dropped from 45 to 12. clinically, the epigastric was tender, but lungs are clear. i didn't wait any longer. while giving the fluid bolus, i made the phone calls.

initially, i referred her to Hospital Selayang. the patient was initially accepted but due to unavailable ICU beds, they couldn't accept her. So did Hospital Sg Buloh. you may think that it's very easy looking for ICU bed kan but trust me, i don't how i managed to sum it up in a line. haha! it's really time consuming, and it's not easy to get through the person that you need to talk to. sometimes, the operator just hentam connecting the lines which really shot my BP up!

it took me almost 3 hours to get the ICU bed, which i managed to get in HTAR, Klang. when we told the father that his daughter need to be sent to Klang, he asked for Selayang or Sg Buloh for logistic reason. we told him again that they are full, and our priority is to save his daughter's life. betul la kan? he looked satisfied with our explanation and went quiet. so unexpected when he threw us with the letter. maybe it's not him, maybe it's the kepochi relatives..but i don't give a damn!

i knew i did the right thing. nothing to worry about, nothing to lose from my side. i did no harm, and my intention was good. but when you threw shit to me, i couldn't stop myself to label you as ungrateful person. period.
